Drive mental health recovery with Northern Health as a Social Worker focusing on specialized outreach. This position requires a Registered Social Worker to deliver clinical services to clients with serious mental illness and substance use disorders.
Under the supervision of a manager, you will conduct a variety of psychosocial assessments, support families, and educate healthcare professionals.
Your role will involve coordinating care alongside an interdisciplinary team, ensuring effective discharge planning and continuity of care. Key Responsibilities:
- Conduct comprehensive psychosocial assessments for clients
- Provide individual and group counseling services
- Liaise and advocate for clients with external resources
- Develop and deliver psycho-educational support groups
- Contribute to discharge planning and case coordination
Requirements:
- Bachelor's or Master's degree in Social Work
- 2 years’ experience in mental health and substance use
- Current BC College of Social Workers registration
- Valid Class 5 BC driver's license
- CPR and First Aid certification
