Urgent Response Service (URS) Coordinator for both Haliburton, Kawartha, Pine RIdge (HKPR) and Durham Region service areas
Contract up to 12 months with possibility of extension
As an Urgent Response Service (URS) Coordinator, you will provide timely, person-centered support to children and youth with Autism and their families. You will assess eligibility for the Urgent Response Service, guide families in accessing appropriate support, develop, monitor, and evaluate individualized service plans. Work collaboratively with families and community service delivery partners, to ensure coordinated service delivery and positive outcomes for children and youth and families.
Responsibilities:
- Screening and determining eligibility for the Urgent Response Service.
- Providing service navigation and referrals to appropriate community resources.
- Gathering and maintaining comprehensive information to support effective service delivery.
- Developing, implementing, monitoring, and updating individualized URS plans.
- Coordinating services and fostering a collaborative experience for children, youth, and families.
- Building strong partnerships with community agencies and URS Service Providers.
- Acting as a liaison and advocating within the community.
- Maintaining accurate documentation and supporting agency policies and objectives.
- Demonstrating strong communication, collaboration, and problem-solving skills.
- Participating in ongoing professional development and continuous learning.
- Performing other related duties as required.
Qualifications:
- Undergraduate degree or diploma in social sciences or related field. Diploma in relevant field with comprehensive experience may also be considered.
- Minimum of 5 years’ experience in the children’s service system.
- Extensive knowledge of the range of support services available to children and their families in Durham Region and beyond.
- Ability to assess needs of a client and make sound judgments to coordinate appropriate and meaningful activities and/or supports.
- Demonstrated ability to effectively support families experiencing significant emotional distress, systemic barriers and identify risk situations requiring prompt action.
- Exceptional leadership, team facilitation, conflict management, negotiation, and mediation skills that support the development and maintenance of good working relationships.
- Good case management/service coordination and organizational skills to support planning for client services.
- Ability to work independently and participate as a team member.
- Apply strong communication skills across a range of audiences and settings (written, verbal, listening and non-verbal), adapting approach and tone with sound judgement and maintaining composure in high-pressure or sensitive interactions.
- Willingness to work flexible hours as required.
