and Clinical supportTravel to LTC homes and retirement communitiesBuild and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnershipsComplete medication reviews in currently serviced retirement communities and long-term care homes serviced by CareRx PharmacyComplete all required documentation and billing processes as required by MOHLTC, Alberta College of Pharmacy (ACP), and other federal and provincial standards and regulationsProvide education to Residents and staff on various health-related topics including the MedsCheck program and attend community-based wellness fairs and other events as requiredConduct education sessions within Long Term Care Homes and Retirement Communities within scope of practice, as applicable (e.G. policy and procedure, web portal, eMAR, influenza protocol, etc.)Support CareRx Pharmacy policies and procedures within retirement communities and long-term care homesComplete medication audits in office in LTC Homes and Retirement Communities as requiredEnsure regular on-site visits are made to all assigned clientsAttend/participate in annual professional conferences and trade shows, when applicableParticipate in the development and implementation of industry-leading clinical pharmacy service, when applicableBe an education, policy and pharmacy initiative resourceBe available for special clinical team projects as requiredComplete/collaborate in the develop of medication audit follow-up improvement plans when applicableWorks collaboratively with other team members to support applicable homes and communitiesParticipate in pharmacy-related transition processes (e.G recent client transitions, eMAR transitions, etc.)Help with pharmacy operations/drug distribution when requiredAttend and actively participate in clinical team meetingsWhat You Will Bring to The TeamInsured and currently licensed in good standing with the Ontario College of Pharmacists, or College of Pharmacists of British Columbia ( RPhT ), or Alberta College of Pharmacy (ACP)Proven ability to foster strong partnerships with other members of the healthcare teamEffective organization, planning,
