As part of the Acute Care Transitions Program, you will be instrumental in enhancing patient transitions from hospital to home. You will work in a self-directed team, conducting assessments and implementing individualized treatment programs. Focus on compassionate care while collaborating with interdisciplinary teams to drive meaningful health improvements for clients in the community.
Key Responsibilities:
Conduct comprehensive assessments for clients
Develop tailored goals and recommendations
Implement client-centered treatment plans
Collaborate with interdisciplinary team members
Provide compassionate support to clients in their home setting
Requirements:
Graduate of an accredited Physiotherapy program
Member in positive standing with the College of Physiotherapists of Ontario
Recent experience in acute or community rehabilitation
Robust interpersonal, organizational, and communication skills
Valid driver’s license and ability to travel
