Description
Hours of work: 8 hours flex, 4 days per week, no weekends (60 hrs. biweekly) - (Balwin Villa & Grand Manor CSW)
What You’ll Do:
Scheduling, preparing, supporting, accompanying, and assisting clients to make and attend professional appointments, activities, community outings, trips, programs, etc.
To be aware of and implement as required Excel Society’s Safety Emergency Response Procedures. Responding appropriately to emergency situations and reporting incidents and accidents within appropriate time and document as per policy.
Supporting clients to work towards their Individual Support Plan Goals in alignment with their Care Plan.
Encouraging and supporting client's efforts to maintain/enhance their health and wellness and assisting client with the maintenance of a clean, healthy, and safe home setting including working with hoarding behaviours.
Completing, reading, and maintaining all required documentation and educational requirements that align with Excel’s requirements and those of our funders and auditors.
Demonstrating exceptional and professional verbal and written communication skills including being skilled in the use of computer use, scheduling, and email.
Able to effectively collaborate with all members of the interdisciplinary team to ensure continuity of care is met and concerns are notified to the appropriate health care provider.
Recognizing preventative safety initiatives including follow-up procedures of occupational hazards and following guidelines for the prevention of fire and electrical accidents.
Respecting client’s rights to confidentiality, privacy, dignity, independence, well-being, and choice.
Understands the policy and procedure for recognizing and reporting signs of client abuse.
What You Bring:
A caring, reliable, and person-centered approach
Strong communication skills - ASL proficiency is preferred
Valid First Aid-CPR Level C Certificate is a mandate.
Valid police information check with vulnerable sector is mandate.
Willingness to follow established policies, procedures, and support plans
Basic computer skills are required
Previous experience in disability services or human services is an asset
