Reporting to the Provincial Manager of the MHA Program, the Health Care Social Worker engages in clinical triage, risk assessment, and brief counselling. You will assist clients in navigating the healthcare system while addressing obstacles to their care and linking them to community resources. This role requires robust communication and motivational interviewing skills to effectively support individuals with mental health and substance use challenges.
Key Responsibilities:
• Facilitate intake for Mental Health and Addictions services
• Utilize screening tools for effective triaging
• Provide brief counselling and psychoeducation
• Build relationships with clients to solve problems
• Act as a liaison with community resources
Requirements:
• Bachelor’s degree in Social Work
• Registration with Nova Scotia College of Social Workers
• Three years of licensed experience in mental health
• Knowledge of harm reduction strategies
• Competencies in other languages preferred
