As a Navigator, you will conduct interRAI CHA assessments, renew service agreements, and manage intake processes for consumers while supporting their access to community resources.Key Responsibilities:Complete interRAI CHA assessments for recent consumersRenew annual agreements for existing clientsOversee intake and referrals effectivelyLiaise with community and agency stakeholdersManage waitlists and maintain organized filesRequirements:Post-secondary education in health/human servicesExperience with interRAI CHA assessments preferredMust have current CPR/First Aid certificationExcellent communication and organizational skillsValid Driver's License with reliable transport necessaryUtilize your skills in this impactful role and support community members in their journey with March of Dimes Canada.
