As a Care Coordinator (Full-Time, Permanent; Start date: Immediately; Location: Sudbury) , you will assessand determine patient care needs and eligibility, provide access and referralsto community services, and engage with patients, caregivers and other healthcare practitioners.

Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centered care – and be supported by our collaborative team thatincludes over 9,000 regulated health care and other professionals.

A s a valuedteam member, yourmission will be to help our patients be healthier at home, while you benefit from oursupports for professional growth, personal wellness and work-life balance.

What will you do?

- In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans
- Linkpatients with service providers
- Coordinate and monitor care plan delivery
- Establish a helping relationship with patients and their families
- Balance patient needs andchoices with available resources,



ensuring patients’ values and preferences arerespected
- Initial assignment Long Stay, withpossibleassignmentchanges within the occupation based on operational needs

What must you have?

- Membership, in positive standing, with the applicableregulatory body:
- College of Nurses of Ontario
- College of Physiotherapistsof Ontario
- College of OccupationalTherapists of Ontario
- Ontario College of SocialWorkers and Social Service Workers
- Recentexperience in community health or a related field
- Knowledgeof the health care delivery system and community resources
- Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
- Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
- Establishedability to accurately complete required documentation, reports and forms
- Avalid driver’s license and access to a reliable vehicle
- Advanced oral and writtenproficiency in English essential
- We have amandatory COVID-19 vaccination policy. As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination statusprior to start date.

What would give you the edge?

- Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
- Casemanagement experience or recent related community experience




- Abilityto speak French or another second language

Salary

Hourly wage of $41.29 to $51.13
