Become an integral part of facilitating the Adult Day Program at Prairie Mountain Health as a Worker focused on activities designed for participant engagement. This role champions quality care and program excellence.Reporting to the Care Team Manager, you'll develop and evaluate recreational and therapeutic activities. This position requires collaboration in a multidisciplinary team to ensure that individual participant needs are identified and met through agile programming while maintaining safety and compliance.Key Responsibilities:Assess and develop customized activity plans for participantsCollaborate with staff on program evaluation and planningMaintain program records, audits, and statisticsCoordinate safe transportation and meal servicesFollow safety protocols and train staff during emergenciesRequirements:Completion of Grade 12 educationAt least 2 years of experience in healthcareProficient in Microsoft applications and information technologyValid Class 5 Driver's License and access to a vehicleStrong ability to work independently and foster communicationEngage with and support participants in the Adult Day Program, enriching their day-to-day experiences at Prairie Mountain Health.
