Key Responsibilities:
• Conduct client assessments for personalized care plans
• Collaborate with healthcare professionals for primary care




• Implement wound care and chronic disease management
• Manage IV therapy and phlebotomy procedures
• Administer palliative and end-of-life care

Requirements:
• Licensed as a Registered Nurse in your province
• Experience in community health is advantageous
• Personal vehicle with current driver's license required
• Satisfactory Criminal Records Check necessary
• Ability to wear Personal Protective Equipment (PPE)
