As a Nurse Case Manager, you will play a vital role in supporting clients with complex health needs. Your expertise in chronic disease and self-care management will be essential. Coordinate care, conduct assessments, and liaise with healthcare professionals to ensure continuous care for your clients.

Key Responsibilities:
• Manage chronic illness care for selected client populations
• Conduct comprehensive assessments and identify care variances
• Liaise with multidisciplinary team and community agencies
• Coordinate clinical interventions and evaluate client outcomes




• Provide education and resources based on nursing theory

Requirements:
• Registered Nurse or Registered Psychiatric Nurse in BC
• Three years' related experience in case management
• Valid BC Driver's License for local area travel
• Deep knowledge of intensive case management principles
• Demonstrated ability to collaborate in a team setting
