Reporting to the Continuing Care Manager, the Registered Practical Nurse will assess, plan, implement, and evaluate nursing care within a multidisciplinary team. You'll care for post-surgical patients, those suffering from cardiac issues, strokes, musculoskeletal problems, chronic illnesses, and dementia. This position requires strong organizational skills and the ability to manage stress in a dynamic healthcare environment.

Key Responsibilities:
• Assess and evaluate patient care plans
• Implement nursing support for diverse patient caseloads




• Collaborate within a multi-disciplinary healthcare team
• Maintain detailed communication and documentation
• Uphold SHN’s safety programs and practices

Requirements:
• Certification from the College of Nurses of Ontario
• Basic Cardiac Life Support competence
• Adult medical/surgical nursing experience
• Solid communication and interpersonal skills
• Ability to work independently in demanding settings
