WoodGreen Community Services in Toronto is hiring for the Cummer Housing Program, Supportive Housing - Community Care Unit. This full time role is on-site at 185 Cummer Ave, Monday–Friday, 9am–5pm, 37.5 hours/week, with a salary around CAD 72,298.50.
You will perform direct client care, psychosocial assessments, crisis intervention, and service coordination, while advocating for clients and coordinating with health and community partners to promote health, housing, and social outcomes.
