This role focuses on providing virtual nursing and patient education, particularly related to menopause and women's wellness. The ideal candidate will have robust communication skills and at least three years of nursing experience in Canada. You’ll collaborate with healthcare providers to support individualized care plans and improve patient health outcomes.

Key Responsibilities:
• Serve as primary contact during patient care journeys
• Provide virtual case management via phone and digital tools
• Conduct assessments to identify patient needs and goals




• Develop individualized care plans with healthcare teams
• Educate patients on health, wellness, and preventative practices

Requirements:
• Current RN registration in Canada with licensure in two provinces
• Minimum of 3 years recent nursing experience
• Strong assessment and patient education skills
• Experience with EMRs and healthcare technology
• Tech-savvy and capable of working independently
