This part-time role involves 4 hours per week of direct patient care, focusing on rehabilitation and wellness. You'll develop evidence-informed care plans and collaborate with fellow healthcare professionals to ensure optimal patient outcomes.

Key Responsibilities:
• Develop personalized rehabilitation and wellness care plans
• Collaborate with a multidisciplinary team for patient care
• Mentor and lead junior staff as assigned




• Stay current with best practices through continuous learning

Requirements:
• Active license in positive standing with regulatory college
• 1–2+ years of outpatient LTC experience preferred
• Exceptional interpersonal and communication skills
• Valid Vulnerable Sector screening check required
• Proof of Tuberculin clearance needed
