At the City of Toronto, our Seniors Services and Long-Term Care Division is looking for a dedicated Rehabilitation Assistant. This role involves assessing residents’ needs, developing rehabilitation programs, and collaborating with multidisciplinary teams. You will focus on promoting functioning and preventing or delaying loss in residents’ abilities.

Key Responsibilities:
• Assess residents' rehabilitative needs through observation
• Develop and implement rehabilitation programs for residents
• Conduct treatment programs for ambulation and mobility




• Monitor resident progress and adjust treatments accordingly
• Document assessments and progress using PCC software

Requirements:
• Completed post-secondary program in restorative care
• Experience in Long-Term Care or Geriatric settings
• Knowledge of physiology and disabilities related to elderly individuals
• Strong verbal and written communication skills
• Ability to work various shifts, including evenings and weekends
