As a Care Coordinator (Full-Time, Startdate: Immediately ; Location: North Bay) , you will assess and determine patientcare needs and eligibility, provide access and referrals to community services,and engage with patients, caregivers and other health care practitioners.
Whether you work in our office, in thecommunity, or a health care facility – you will play a lead role in providingconnected, accessible, patient-centered care – and be supported by ourcollaborative team that includes over 9,000 regulated health care and otherprofessionals.

What will you do?
In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans
Linkpatients with service providers
Coordinate and monitor care plan delivery
Establish a helping relationship with patients and their families
Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected
Initial assignment Long Stay, with possibleassignment changes within the occupation based on operational needs
What must youhave?
Membership,in positive standing, with the applicable regulatory body:
College ofNurses of Ontario
Collegeof Physiotherapists of Ontario
Collegeof Occupational Therapists of Ontario
OntarioCollege of Social Workers and Social Service Workers
Recentexperience in community health or a related field
Knowledgeof the health care delivery system and community resources
Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
Establishedability to accurately complete required documentation, reports and forms
Avalid driver’s license and access to a reliable vehicle
Advancedoral and written proficiency in English and French is essential
Wehave a mandatory COVID-19 vaccination policy. As a condition ofemployment, all employees are required to submit proof of COVID-19vaccination status prior to start date.

What would give you the edge?
Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
Casemanagement experience or recent related community experience
Abilityto speak French or another second language
French Proficiency: French an asset
Salary: Hourly wage of $41.29 to $51.13
Terms of Employment: Full-Time
