As a home health aide at CenterWell, you will play a critical role in patient recovery and daily living assistance. This position requires direct patient care under RN supervision, managing self-administered medications, monitoring vital signs, and ensuring optimal personal hygiene. You'll engage in activities that significantly improve the quality of life for patients while documenting care and changes accurately.

Key Responsibilities:
• Assist patients with self-administered medications and daily routines
• Monitor vital signs and communicate changes to supervisors
• Ensure patient hygiene through bathing,



grooming, and oral care
• Support patients with mobility and exercises per care plans
• Document patient interactions and care provided accurately

Requirements:
• Certified Nursing Assistant or Home Health Aide within last 24 months
• Valid driver’s license and reliable transportation
• Minimum one year of relevant experience in healthcare
• Positive standing on HHA Registry, if applicable
• Ability to follow infection control protocols
