Key Responsibilities:
• Establish therapeutic relations with clients through effective communication
• Conduct thorough assessments and create individualized care plans
• Collaborate with healthcare providers to address client care needs




• Deliver direct nursing care and health education in various settings
• Supervise and delegate tasks to Community Health Workers

Requirements:
• Current Registered Nurse registration with BCCNM
• One year of recent clinical experience in community health
• Valid BC Driver's license and personal vehicle access
• Robust knowledge in gerontology and chronic disease management
• Ability to work independently or as part of a team
