Provide holistic primary health care at CMHA Toronto as a part time Nurse Practitioner in North York. Engage with clients through assessments, diagnoses, and treatments in a welcoming clinic environment.
This permanent 0.5 FTE position supports your work-life balance through adaptable scheduling. As a Nurse Practitioner, you’ll use your expertise in health assessment, chronic disease management, and collaborative care to ensure comprehensive service delivery. Your role will also enhance community well-being through health promotion and education initiatives.
Key Responsibilities:
• Conduct comprehensive health assessments across age groups
• Diagnose and monitor acute and chronic conditions
• Prescribe medications and manage treatment plans
• Provide health promotion and disease prevention education
• Collaborate with multidisciplinary team members
Requirements:
• BScN Degree and Ontario nursing registration
• Minimum 5 years in community mental health practice
• Experience collaborating in care teams
• Strong decision-making and critical thinking skills
• Valid Ontario driver’s license with vehicle access
