Become a Concurrent Disorders Specialist with CMHA Toronto's Flexible Assertive Community Treatment (FACT) Team. Join a mission-driven team to support adults with mental health and substance use issues.
In this full time permanent role, you'll conduct assessments and create personalized recovery plans for individuals facing serious mental illness and concurrent substance use. Equipped with evidence-based practices, you will engage clients, provide treatment, and facilitate their access to community resources. Your expertise will also guide the multidisciplinary team towards better client outcomes.
Key Responsibilities:
• Conduct comprehensive assessments for recovery-oriented service plans
• Provide evidence-based treatment and harm reduction strategies
• Build therapeutic relationships to foster client engagement
• Facilitate group education for clients and supporters
• Collaborate with various healthcare and community organizations
Requirements:
• Bachelor's degree in Social Work, Psychology, or related field
• At least three years of community-based experience
• Knowledge of trauma-informed practice and addiction treatments
• Strong crisis intervention and assessment skills
• Valid Ontario Driver's Licence for community outreach
