Key Responsibilities:
• Perform psychosocial assessments for inpatient units
• Provide case management and discharge planning

Requirements:
• Bachelor's degree in Social Work required
• Active registration with ACSW or eligibility
• 3 years' experience in an adult acute care setting
• BCLS-HCP certification needed
• Solid problem solving and communication skills required
