As a key member of the interdisciplinary team at SOAHAC, the Harm Reduction Care Coordinator will offer client-centered services. Key Responsibilities:
• Provide harm reduction support and care coordination
• Conduct intake assessments and wellness planning

Requirements:
• Post-secondary degree in health or related field
• Minimum one year of relevant experience
• Knowledge of Indigenous harm reduction philosophies
