The salary range for this position is CAD $32.84/Hr. - CAD $41.35/Hr.The hours of work, including start and stop times, days off, work area and work site may be subject to change consistent with operations requirements and applicable provisions of the Collective Agreement and statutes. Testing may be required. A criminal record check may be requiredReporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia.The LPN functions as an independent practitioner, while recognizing situations such as increased patient acuity or care that falls outside the scope of an LPN as appropriate for accessing the RN/RPN for collaborative support and/or clinical intervention. The LPN assists patients/residents in meeting their physical, social, spiritual and psychological needs consistent with the mission, vision and values of Providence Health Care (PHC) and established protocols.Participates in developing care plans, conducting assessments, implementing and evaluating interventions, documenting information and acting as an advocate for patients/residents. Participates in a variety of meetings including care conferences, and assists with orientation of new staff and students as required. Responds and reports incidents according to established procedures and performs clerical duties as needed.Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. individualized care plans, and prioritiesConduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as neededPrepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary teamAdminister medications per protocol, monitor effects, and maintain communication with the teamUtilize evidence-based resources to stay updated on nursing practicesRespond to incidents such as codes, injuries, spills, and alarms, following proceduresAssist with staff orientation, demonstrating procedures and providing supportAddress patient/resident and family learning needs, provide educational materials, and make referralsAttend care planning meetings, provide input, and advocate for patients/residentsCollaborate with the team to develop and evaluate individualized care plansAssist with discharge planning based on patient status and external supportsOperate and clean equipment, report malfunctions, and handle hazardous materials.
