Salary Range: $32.84 To $44.96 Hourly

Aboutthe Role The Licensed PracticalNurse (LPN) reports to the Director of Care and/or designate. This position isthe role model and team leader to an assigned comunity,



and responsible for coordinatingcare delivery of the persons served. The LPN performs duties and monitorsdepartmental activities in accordance with their scope of practice, establishedstandards, regulations, policies and procedures to maximize individualized andresident-centered care.
Responsibilities Include but not limited to:
Be an engaged cooperative team player, developing andmaintaining professional relationships that support and strengthen theorganizational culture and purpose.
Assesses residents’ needs; implements and evaluates individualized resident-centered care plans.
Ensures residents rights are observed and adheres to aholistic philosophy of wellness.
Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centred care is delivered.
