Contribute to mental health support as a full time Clinical Navigator at PAARC's Brampton HART Hub. Your role involves client assessments, immediate interventions, and facilitating access to essential services.

This position is ideal for professionals experienced in mental health, substance use, and community resources. You will conduct thorough assessments and support clients through short-term interventions, including crisis de-escalation strategies. Regular collaboration with a multi-disciplinary team is essential for promoting community awareness and seamless service connections.

Key Responsibilities:
• Conduct comprehensive client assessments for service eligibility
• Provide immediate, task-based interventions to clients




• Facilitate connections to other services within the HART Hub
• Promote awareness of HART Hub services in the community
• Update documentation accurately within 24 hours

Requirements:
• Post-secondary degree or diploma in a related field
• Minimum 2 years’ experience in mental health and addictions
• Valid Ontario Driver’s License and reliable vehicle
• Current CPR and First Aid certifications required
• Proficient in prevalent languages in the Region of Peel
