Key Responsibilities:
• Conduct comprehensive diabetes assessments for patients
• Develop integrated care plans in collaboration with healthcare teams




• Lead patient education sessions in diverse settings
• Evaluate and document nursing care for individuals and families
• Support self-management strategies for complex diabetes cases

Requirements:
• Bachelor of Science in Nursing (BScN)
• Minimum 2 years of related experience
• Current registration with the College of Nurses of Ontario
• Certified Diabetes Educator
