Ontario Health atHome is seeking a Care Coordinator (Full time, Permanent) for North Bay or Kirkland Lake. You will assess patient care needs, determine eligibility, and connect patients with community services, while engaging with patients, caregivers and health care practitioners.
You will play a lead role in providing patient-centred care across office, community, or health care facility settings, supported by a large, multidisciplinary team.
