Become a pivotal Case Manager with Six Nations of the Grand River, focusing on the reintegration of clients with concurrent disorders. This full time, on-site role requires collaboration with the Mental Health and Addictions team to support individuals transitioning back to the community.
As the Case Manager for Concurrent Disorders, you will work closely under the supervision of the Mental Health and Addictions Manager. This role demands strong assessment and counseling skills to assist adult clients who face mental health challenges and substance abuse issues. Your impact will be significant in providing culturally sensitive community programming aimed at successful client reintegration.
Key Responsibilities:
• Conduct initial screenings and risk assessments for clients
• Collaborate with community health teams for integrated care
• Provide crisis support and mental health counseling
• Supervise student placements in related programs
• Maintain detailed documentation per policy standards
Requirements:
• Degree in social work or psychology; or diploma with 2 years' experience
• Valid Class G driver's license and vehicle required
• High regard for confidentiality and discretion
• Knowledge of mental health and criminal justice systems
• Experience with substance abuse clients beneficial
