You will support seniors facing homelessness with housing, health, and social services, promoting independence and well-being.You will build relationships, assist with care plans, crisis support, ADL/IADL assessments, and connect clients to community resources. Solid communication and de-escalation skills are essential, with 2+ years of relevant experience preferred.
