As a Care Concierge, your daily tasks will include enrolling members, conducting welcome visits, and coordinating care navigation to address members' needs effectively. This position demands a strong understanding of healthcare processes and relationship-building with older adults while flexibly adapting to real-time demands.

Key Responsibilities:
• Conduct outreach to prospective members and gain trust
• Perform welcome visits for newly enrolled members

Requirements:
• 1+ years of member-facing healthcare experience
• Solid communication skills with older adults
