Centretown CHC seeks a Diabetes Educator (Nurse or Dietitian) to deliver and promote its Community Diabetes Education Program across Ottawa. You will provide group and individual education to adults with prediabetes or type 2 diabetes, support starting insulin, and assist with titration and chronic disease management.

Collaborating with physicians and endocrinologists, you will work from multiple Ottawa locations, participate in outreach activities, and engage with diverse communities, including
