Durham Community Health Centre is seeking a Case Manager for the Welcoming Streets program. Focus on outreach and case management for marginalized communities in Oshawa. This position is a temporary full time role aimed at providing individualized support using the Housing First Model.
Your work will involve collaboration with medical professionals to enhance health outcomes for individuals facing barriers such as homelessness and mental health issues across Durham Region. Provide outreach services in community locations
- Assist clients with accessing housing and financial resources
- Establish partnerships with local service providers
- At least 2 years of experience in community outreach
- Relevant post-secondary education in social services
- Make a difference in the lives of vulnerable individuals as a Case Manager at DCHC.
