New or Replacement Position : Replacement Salary: $49.86 - $62.32 per hour Shifts: Days Temporary Full-Time (1 year) As a key member of the Toronto Western Site and UHN Wound and Skin Clinical Leadership interdisciplinary team, the Clinical Nurse Specialist, Wound, Ostomy, and Continence Care, with a specialty-based focus, will function in an advanced nursing role, demonstrating a high level of autonomy and clinical expertise to formulate decisions, manage acute and chronic conditions, and promote wellness for patients and their families. This role involves advanced clinical decision-making and leadership in nursing practice.
The Clinical Nurse
Specialist serves as a leader and change agent, supporting the implementation of best practices and the advancement of nursing standards Approximately 60% of the role will have aligned focus on developing a comprehensive program for patients within Krembil Neurosciences (Movement Disorders) requiring percutaneous endoscopic gastrostomy tubes including outreach to community partners both locally and nationally.
Utilize and demonstrate comprehensive theoretical knowledge base and advanced level of clinical competence in caring for patients with complex wounds, including collecting and interpreting data about the health of the patient, managing complex responses of individuals and families to actual or potential problems
• Build point of care staff capacity to manage complex wound, ostomy and continence issues
• Play a leadership role and perform quality improvement activities
• Participate in policy and procedure development
• Act as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies
• Current registration with the College of Nurses of Ontario and able to practice nursing without restrictions Completion of a masters degree in Nursing or recognized equivalent Five (5) years experience in a tertiary care or complex continuing care setting Experience in research &
