Department Profile The Stellat’en Health Centre focuses on community health holistically. Supported by Carrier Sekani Family Services through the First Nation Health Authority. Provide members access to health care professionals (doctors, nurses, specialists), health services (mammograms, eye exams, A1C testing for diabetes), counselling (mental wellness, drug & alcohol), and health programs (Health Fair, men’s, women, youth programs).
The Health
Centre runs the community garden and provides produce to the community to promote good health through proper nutrition. Provide equality & confidentiality to all our clients & staff and strive to make everyone feel they are always in a safe & healthy environment.
The First Nation Health
Benefits (FNHB) program offers services to Stellat’en Members registered as Status Indians. This program helps clients apply for and access funding for healthcare and healthcare-related travel expenses provided by First Nation Health Authority. The program ensures clients have the necessary medical and administrative documents they need to apply for reimbursement of certain healthcare cost by the Canadian Federal government. This program aims to make healthcare services more accessible and to facilitate optimal health and well-being.
Position Summary The Community Health Representative is responsible for participating in the delivery of high standard Community Health, treatment and surveillance programs to provide quality heath prevention and treatment in the Stellat’en First Nation community.
The Community Health
Representative directly impacts individuals and the community by working with health care providers and the community to educate and provide information and support to individuals and families in the Stellat’en First Nation.

Representative works closely with the funding agency, First Nation Health Authority, Community Health Nurse, Health Care Professionals and key Stellat’en First Nation staff. A strong holistic and traditional healing approach is used. This position directly reports to the Health Director/General Manager.
Major Accountabilities
- Liaise between patients, families, the Stellat’en First Nation community,
and health care providers to ensure patients and families fully understand their conditions and treatment, and receive appropriate care
- Educates and monitors the community for general health
- Plans and delivers health education and health prevention services by providing an annual list and description of community health education priorities to the Community Health Nurse and program funding agencies; preparing and submitting quarterly active reports; liaising with the Community Health Nurse, social development, and health food workers
- Provides health detection support services for all members of Stellat’en First Nation by working with the Community Health Nurse to monitor prenatal, sexually transmitted infections, diabetes, TB and other health events requiring detection, monitoring and possible intervention
- Delivers needed health care assistance and support services to members of the Community, including Patient Travel Clerk duties for the FNHB Program.
- Provides consulting and support services to individuals and groups that encourage prevention and intervention when necessary
- Provides up-to-date information and resources to promote healthy lifestyles through education, immunization and clinics to the members of Stellat’en First Nation
- Promotes cross-cultural understanding between Stellat’en First Nation clients and health care professionals from outside agencies
- Oversees TESI or PGNAETA sponsored summer student employment initiatives
- Attends and represents Stellat’en First Nation health interests on community committees and networking events
- Liaise with Chief and Council and the General Manager on community Health issues
- Performs other duties as required
Skills, Knowledge & Abilities of Position
- Strong understanding of crisis management and the ability to deal with life threatening situations,
including the prevention of suicide
- Knowledge of individual and community health legislation, theories, practices and procedures, and Traditional Medicine
- Ability to manage and maintain patient records and files
- Working knowledge of FNHB Patient Travel guidelines
- Ability to use electronic charting and data storage
- Ability to network effectively in the community and with health care professionals
- Possess a working knowledge of client/patient health planning

- Willingness to adhere to a confidentiality contract
- Strong knowledge of the Stellat’en First Nation culture and traditions
- Initial and on-going employment is subject to submission of Criminal Record Check and expected to sign SFN Code of Conduct &Ethics; and SFN Oath of Confidentiality Contract
Education and Experience Requirements
- A Bachelor of Science in Nursing or completion of a recognized Community Health Representative Program is considered an asset
- Possess a Community Health Representative Certificate
- A minimum of a high school diploma is required
- A minimum of two years’ work experience providing health support services
- Possess a valid CPR and/or Level 1 First Aid Certificate
- Valid Class 5 Driver’s License and reliable vehicle
- Case management and confidential record management of individuals' files
- Experience in working with First Nations Communities
- An equivalent combination of education and experience may be considered
Job Type: Full time Pay: $20.00-$30.00 per hour
