Permanent
Part-time Hours of Work: Available to work Monday to Friday, full days with flexibility to meet patient needs.
Location: Humber River Health, Church (Subject to change) Manager, Dialysis (Church) Hourly Rate: $41.68 - $53.28 As a member of the health care team, this position assumes the responsibility for assessment, planning, implementation, care coordination and evaluation of patients who referred for diabetes care and management. Clinical supervision of clinical nutrition students/interns. Bachelor’s degree in Food and Nutrition Sc. in Nutrition or related field preferred Completion of an accredited dietetic internship program and a member (or eligibility to become a member) of the Dietitians of Canada.
Must be a member in good standing with the College of Dietitians of Ontario.
Excellent knowledge of clinical nutrition principles and trends.
Experience in applying principles of adult education in ambulatory care and the ability to teach in an individual or group setting. Knowledge of community resources and experience in community outreach preferred. Recent experience with Nephrology/Dialysis and chronic illness is preferred.
Demonstrated excellent written and verbal communication skills, including an ability to encourage, support and motivate patients and/or their families from a multi-cultural community Computer skills required; Demonstrated ability to work independently and as a member of the healthcare team. Excellent communication skills (oral and written) facilitating effective communication with patients, peers, other healthcare team members and volunteers. Demonstrated ability to conduct research in order to develop resources/services and improve policies affecting the program.
Excellent interpersonal skills with an ability to cultivate and maintain mutually supportive working relationships with other members of the health care team Ability to speak a second language an asset. Must be proficient in Microsoft Word, Access, Excel, etc. An understanding of equity, diversity, and inclusion principles as they relate to health practices is an asset.
