Health Sciences North is seeking an Extended Length of Stay (ELOS) Risk Assessor to support the Hospital to Home program. You will complete risk screens, work with interdisciplinary teams, and determine eligibility for patient pathways.

Responsibilities include on-boarding patients, liaising with community services, and supporting discharge planning while aligning with the organization’s strategic goals.
