Function as a Case Manager for a designated group of program participants and as a liaison between the participant and their primary care physician and other health care providers Provide motivational support to participants and families regarding long-term behaviour modification BSc Kinesiology, Physical Education, Physiotherapy or related field from a recognized university, with successful completion of cardiac physiology, exercise physiology electives and cardiovascular rehabilitation elective Eligible for membership with Cardiac Rehabilitation Network of Ontario and Canadian Association of Cardiovascular Prevention & Rehabilitation Cardiopulmonary exercise testing experience preferred Current ACSM Clinical Exercise Specialist and CSEP CEP certification preferred Minimum of 2 years relevant experience in cardiovascular rehabilitation preferred Solid patient assessment skills and program planning experience Experience in adult education and presentation skills Working knowledge of Microsoft Suite Demonstrated commitment to customer service and teamwork Southlake Health is fully committed to a culture of belonging and an inclusive environment that encourages every team member to lead within their role,
