You will support medical office staff in adopting these workflows and ensure proper data sharing between community offices and the FAST program. This full-time position requires a strong focus on change management and continuous improvement within the healthcare sector. Key Responsibilities:
- Foster strong relationships with primary and specialty care providers
- Support the adoption of referral workflows in clinics
- Facilitate data-sharing processes with physicians and managers
- Assist in training and onboarding for electronic referral systems
- Participate in quality improvement and stakeholder evaluation
Requirements:
- Post-secondary degree in a relevant field
- Minimum 3 years in progressively responsible healthcare roles
- Previous experience with community Electronic Medical Record (EMR)
- Strong communication and problem-solving abilities
- Valid driver's license and vehicle required
