Senior Home Care by Angels Milton is seeking a compassionate, organized coordinator to help maintain reliable home-care services and strong communication with clients and families. You will be a visible presence in the community, follow up on concerns, and help ensure our PSWs understand each client’s care instructions.

Responsibilities

- Visit clients at home to check on their experience and identify concerns early.
- Conduct spot checks and review reported late arrivals, early departures, and missed visits.
- Speak with clients, families, and PSWs about concerns, document findings, and follow through on agreed actions.
- Introduce new PSWs to clients and explain home routines and written care instructions.
- Review visit records and care notes, escalating documentation gaps or changes in client needs.
- Work with the office scheduler to flag coverage issues and keep families informed.
- Provide a brief weekly report on visits, outstanding concerns, and actions requiring management attention.

Qualifications
- Education or experience in social work, social services, community health,



home care, or a related field.
- Experience supporting older adults and communicating with families.
- Sound judgment, empathy, and confidence handling difficult conversations professionally.
- Strong documentation, organization, and follow-up skills.
- Comfort using scheduling or care-record software.
- A valid driver’s licence and ability to travel within Milton, Oakville, and surrounding service areas. A company vehicle will be available for assigned field visits.

Experience with dementia care, PSW services, client complaints, or staff coaching is an asset. This role focuses on service coordination and quality oversight; hands-on PSW care and clinical social work are not required.
