This full-time position requires you to work closely with participants to develop, implement, and evaluate their individualized housing goals.
Rooted in a harm reduction approach, you will collaborate with various community resources, assist in crisis situations, and prioritize relationship building to ensure participant success. Key Responsibilities:
- Implement individualized care plans with ongoing support
- Identify and address barriers to housing for participants
