Lead patient flow and care as the Inpatient Stroke Service Coordinator at UHN. This temporary full-time role requires solid leadership in patient admissions, discharges, and team coordination. As part of UHN's Brain and Spinal Cord Rehab Program, you'll support multidisciplinary teams and enhance patient care processes.
With a focus on quality improvements, your role will involve organizing and evaluating operations related to patient care. You’ll be instrumental in mentoring staff and acting as a clinical resource for complex cases. Key Responsibilities:
- Implement organizational initiatives and quality improvements
- Plan and oversee patient admissions and discharges
- Provide mentorship and promote a supportive environment
- Consult with teams on clinical decision making
- Maintain strong relationships with patients and families Requirements:
- Master’s degree in health-related discipline
- 4-6 years of practical experience
- Knowledge in admission and discharge planning
- Strong leadership and organizational skills
- Valid Health related College Certificate Utilize your clinical expertise and leadership skills to ensure excellent patient care at UHN.
