Job Summary

We are seeking a proactive and compassionate Intake Coordinator to join our dynamic healthcare team. In this vital role, you will serve as the first point of contact for patients and their families, coordinating initial assessments and ensuring seamless communication between clients and healthcare providers. Your energetic approach will help facilitate timely access to services, support case management processes, and uphold the highest standards of patient care.

Responsibilities

- Act as the primary contact for new patient inquiries, providing transparent guidance and compassionate support throughout the intake process
- Gather comprehensive patient information
- Support our admin team in coordination of appointments, etc.




- Coordinate with clinical teams to schedule assessments, appointments, and follow-up services efficiently
- Maintain detailed records of all intake activities, ensuring confidentiality and compliance with healthcare regulations
- Collaborate with the team
- Support patients in navigating complex healthcare systems

Skills

- Prior experience in a healthcare settings is highly preferred
- Strong understanding of addiction and mental health
- Knowledge of and case management principles
- Excellent communication skills with an energetic and empathetic approach
- Ability to manage multiple priorities efficiently in a fast-paced environment
- Familiarity with healthcare software systems and documentation protocols
