Empower the Black communities as an Intensive Case Manager with CMHA Peel Dufferin. This on-site role focuses on holistic care and requires experience in mental health and addictions support. This permanent full-time position involves providing individualized case management to clients with responsible mental health and addiction issues.

You will facilitate access to primary care and community services, using a stepped-care approach. Additionally, working within a multidisciplinary team, you'll develop care plans and conduct regular assessments. Key Responsibilities:

- Conduct needs assessments and monitor care plans
- Provide task-based interventions related to social determinants of health
- Connect clients to crisis services when needed




- Collaborate with the multidisciplinary team for care planning
- Facilitate client attachment to primary care providers

Requirements:

- Post-secondary degree in relevant discipline
- Minimum 2 years of experience in mental health
- Expertise in community resources and support
