Department: Mobile Acute Care of the Elderly
The Social Work Geriatric Assessor (GA) is a key member of NBRHC’s Mobile Acute Care of the Elderly (MACE) Program, providing specialized assessment and care for older adult inpatients. Using the Comprehensive Geriatric Assessment (CGA) model, the GA conducts multidimensional evaluations and collaborates with physicians, nursing, and allied health professionals to develop individualized care plans that maintain independence, optimize function, and prevent avoidable hospital use. The GA identifies and manages geriatric syndromes, supports early and complex discharge planning, coordinates with community partners, and ensures follow-up for frail or medically complex patients.
QUALIFICATIONS
Education
- Bachelor of Social Work (BSW)from an accredited program is required
- Current registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required
- Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate is required (recent within the last twelve months)
- Current Certificate of Registration in good standing as a Health Professional with an Ontario Qualified Association or regulatory college
- Completion of the Provincial Geriatrics Leadership Ontario (PGLO) Common on-boarding (Levels 1 & 2), or willingness to complete within the year
- Completion of Geriatric Certificate Program (McMaster university) required, or willingness to obtain within 2 years
- Current Gentile Persuasive Approach (GPA) Certification required
Experience/Knowledge/Skills:
- Experience working within a health care setting with a focus on older adults.
Preference is to those with experience within the past 5 years
- Experience or demonstrated understanding of the Comprehensive Geriatric Assessment framework, and geriatric goals of care
- Excellence in analyzing assessment findings and implementing individualized treatment plans based on the Comprehensive Geriatric Assessment framework
- Knowledge of Specialized Geriatric Services across the continuum - including inpatient services, community clinics, programs, and resources
- Knowledge of consent and capacity matters and legislation applicable to this older adult population (e.g. Public Guardian Trustee, Substitute Decision Maker Act, Health Care Consent Act, Mental Health Act, Long-term Care Act, PHIPA)
- Ability to exercise professional judgement and critical thinking, with the ability to apply ethical frameworks, supporting the optimal assessment and treatment intervention
- Skills in management of geriatric syndromes (i.e. polypharmacy, falls, functional decline, acute/sub-acute cognitive changes, bowel/bladder, and nutrition/hydration, etc.)
- Excellence in a leadership role
- Excellent levels of interpersonal skills, written, and verbal communication skills, with consideration of the intended audience
- Demonstrate person and caregiver focused care,
supported by best evidence-based clinical practices within respective scope of practice
- Ability to work both independently and collaboratively with all members of the multidisciplinary team, clients, families, regional support services, and community partners
- Be committed to continuing education on a annual basis and demonstrate commitment to quality improvement
- Basic computer knowledge
- Commitment to ensure personal safety, safety of fellow workers, patients, volunteers, and visitors
- Ability to support and contribute to a culture of safety and prevention of adverse health events
- Good attendance and work record
- Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.
