2026 Job Summary The Pharmacist will be responsible for providing patient-focused care to the inpatient clinical units at West Park Healthcare Centre with cross-coverage to other areas as required. Services include Acute Brain Injury (ABI) Behavioural, Amputee Rehab, Complex Continuing Care (CCC), Geriatric Rehab and Geriatric Low Tolerance Long Duration, Long-Term Ventilation, Musculoskeletal, Respiratory Rehab and Respiratory CCC, Stroke and Neurological Rehab, and Tuberculosis Rehab. this includes identifying, resolving, and preventing drug-related problems by conducting thorough pharmacotherapy assessments, performing medication histories and medication reconciliation, providing drug information to patients and healthcare teams, and other patient care activities Participates in education activities including career development and continuing education; mentorship and training of new pharmacy staff and learners Participates in development, review, implementation, and evaluation of projects and initiatives in line with the priorities and/or objectives of the pharmacy department and/or organization Possess a Bachelor of Science degree in Pharmacy, Doctor of Pharmacy degree or equivalent required Certificate of competency from the Pharmacy Examination Board of Canada required Active registration with the Ontario College of Pharmacists (Part A) required Hospital pharmacy practice experience preferred Clinical experience with patient-centered care in rehab an asset Hospital pharmacy residency certificate an asset Membership in the Canadian Society of Healthcare-Systems Pharmacy an asset Government organization and a member of the Healthcare of Ontario Pension Plan (HOOPP A flexible work environment Additional perks (multiple corporate discounts including: travel, restaurants, parking, phone plans, auto insurance discounts, on-site gyms, etc.) At University Health Network (UHN), artificial intelligence technologies may be used to assist in the screening, assessment, and selection of candidates for this position.
