Role: Primary Care Nurse Practitioner – Youth Wellness (YWHO) Facility: Youth Wellness Hubs Ontario (YWHO) – Community Health Organization, Scarborough Position Type: Part-Time | Permanent (21 hours/week) Salary: $52.83 - $74.83/hour | HOOPP pension, extended health & dental benefits TAPP Workforce Solutions is recruiting a Primary Care Nurse Practitioner on behalf of a community health organization serving diverse populations in Scarborough, in partnership with a youth-focused mental health agency. This role is embedded within the Youth Wellness Hubs Ontario (YWHO) model, providing low-barrier, youth-centred care for individuals aged 12-25. The focus is on mental health, substance use, and holistic primary care within a collaborative, interdisciplinary environment.

Some evenings and occasional weekends are required. Provide episodic primary care to youth (ages 12-25) Integrate mental health and substance use care into treatment planning Apply trauma-informed, harm reduction, and culturally responsive care Collaborate with an interdisciplinary team (therapists, peer support, etc.)



Participate in care planning and case discussions Support health education and system navigation Contribute to quality improvement and program development Master’s degree in Nursing with Primary Health Care Nurse Practitioner certification Registered in good standing with the College of Nurses of Ontario (Extended Class) Experience in community health or primary care settings (preferred) Experience with youth, mental health, and/or substance use (preferred) Technical Skills Commitment to equity, diversity, and culturally responsive care The ideal candidate is a compassionate and skilled Nurse Practitioner who thrives in community-based, interdisciplinary settings.
