In this ambulatory role, you will conduct medication reviews, implement treatment plans, and ensure optimal patient outcomes through comprehensive drug therapy management. The role also encompasses education, documentation, and potential virtual support for multiple clinics. Key Responsibilities:
- Perform comprehensive medication reviews for patients
- Collaborate with healthcare teams for treatment plans
- Monitor patient responses and adjust medication therapy
- Document all direct patient care activities accurately
- Provide drug information and educational resources Requirements:
- Bachelor of Science in Pharmacy required
- Current registration with Alberta College of Pharmacy
- Recent direct patient care experience needed
- Excellent communication and decision-making skills required
