Unit Summary The Palliative Care Clinic

Nurse will provide patient centered care through facilitating access to services and providers for patients who have been referred for palliative care. The individual will work with patients and families/caregivers, and health care providers within the palliative program and within the community to coordinate palliative care. The Nurse will also educate patients/families/caregivers about a palliative approach to care and will provide telephone and clinic in-person assessment, and management of symptoms.

The palliative nurse will work closely with the IP team, including palliative care physicians in the ambulatory and in-patient platform in order to provide quality palliative care. Graduate from an accredited School of Nursing program Registered in good standing with the College of Nurses of Ontario (CNO)



or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO Previous experience in Medical/Palliative is required. Preference will be given to those candidates with recent Palliative Care certification.

Knowledge of the philosophy and concepts of Palliative Nursing practice. Demonstrates excellence in communication skills, strong attention to patient and family centred care and establishing therapeutic relationships The ability to provide nursing care within a primary nursing framework. The ability to clearly articulate nursing assessments and patients' goals of care.
