Carewell is seeking a Care Concierge in Canada to join a growing care navigation program. You will support enrollment, welcome visits, and ongoing care coordination, guiding members across the early journey and closing care gaps with providers and community resources.

In this role you’ll adapt to shifting priorities, document thoroughly, and collaborate with a cross-functional pod to deliver high‑quality, person‑centered care while ensuring compliance and consent gates are respected.
